O99.21- obesity complicating pregnancy on nutrition claims

The short answerObesity in a pregnant patient is coded O99.21- first, with a sixth character for the trimester (O99.211-O99.213), childbirth (O99.214) or the puerperium (O99.215), then an E66 code for the type of obesity. Z68 BMI codes aren't used during pregnancy. UnitedHealthcare's preventive row for healthy weight gain in pregnancy covers MNT with a pregnancy diagnosis code, and O99.21- is on its list.
SubcategoryO99.21, obesity complicating pregnancy, childbirth, and the puerperium (header)
BillableO99.210 unspecified trimester, O99.211 first, O99.212 second, O99.213 third, O99.214 childbirth, O99.215 puerperium
Use additional codeE66.- for the type of obesity (e.g., E66.811-E66.813)
Don't addZ68 BMI codes (not assigned during pregnancy)
Also considerZ3A weeks of gestation; O24.41- if gestational diabetes; O26.0- excessive weight gain
Preventive routingUHC MP.016.59 and Cigna A004 accept MNT with a pregnancy/maternity diagnosis for healthy weight counseling
FY2027 changeNone to O99.21- (Oct 1, 2026)

Prenatal nutrition visits for patients with obesity are some of the most clearly useful visits a dietitian can bill, and some of the most commonly miscoded. The habits that work for every other weight-management claim (E66 first, then a Z68 BMI code) are exactly wrong in pregnancy.

The O99.21 codes

Subcategory O99.21, obesity complicating pregnancy, childbirth, and the puerperium, needs a sixth character. O99.21 itself is a header.

Code Descriptor
O99.210 Obesity complicating pregnancy, unspecified trimester
O99.211 Obesity complicating pregnancy, first trimester
O99.212 Obesity complicating pregnancy, second trimester
O99.213 Obesity complicating pregnancy, third trimester
O99.214 Obesity complicating childbirth
O99.215 Obesity complicating the puerperium

Instructional notes:

Section I.C.4.b.1 of the guidelines confirms the obesity class codes (E66.811-E66.813) can be reported with other obesity codes in Chapters 4 and 15. So O99.212 + E66.812 is a complete pair.

No O99.21 code changed in the FY2027 update effective October 1, 2026.

Pregnancy rules that change the claim

Situation Code Page
Gestational diabetes O24.41- O24.4 gestational diabetes
Excessive weight gain in pregnancy O26.0- (by trimester) R63.5 excludes it; see R63.5 weight gain
Low weight gain in pregnancy O26.1- (by trimester)
Malnutrition in pregnancy O25.1- (by trimester)
Bariatric surgery status in pregnancy O99.84-
Pregnancy incidental to the visit, per the provider Z33.1 instead of Chapter 15 codes Rare for a prenatal nutrition visit

When a patient has both GDM and obesity, both obstetric codes can go on the claim. Sequence the one that's the main reason for the visit first.

Coverage for prenatal weight counseling

This is where O99.21- differs from most weight codes: pregnancy counseling has its own preventive recommendation. The USPSTF gives a B grade (May 2021) to behavioral counseling to promote healthy weight gain and prevent excess gestational weight gain.

The benefit split is explained in preventive vs. medical nutrition benefits.

Example claim lines (invented)

Scenario CPT Diagnosis order
Second trimester, class 2 obesity, 60-minute initial 97802 × 4 A: O99.212, B: E66.812, C: Z3A.20
Third trimester, obesity plus diet-controlled GDM 97803 × 2 A: O24.410, B: O99.213, C: E66.9, D: Z3A.30
Four weeks postpartum, class 1 obesity 97803 × 3 A: O99.215, B: E66.811
Eight months postpartum, class 1 obesity, BMI 32 97803 × 3 A: E66.811, B: Z68.32

Notice what's missing from the first three rows: a Z68 code.

What the note should show

Denials and fixes

What the remit says Usual cause Fix
Rejected: invalid diagnosis O99.21 billed without the sixth character Add the trimester, childbirth or puerperium character
Denied or edited Z68 code on a pregnancy claim Remove the BMI code; corrected claim
CO-11 (inconsistent with procedure) E66 first with no pregnancy code, on a preventive pregnancy row O99.21- first, E66 second
Processed under the medical benefit instead of preventive Maternity code not in the primary position (Cigna) Corrected claim with the maternity code first, if documentation supports it
CO-119 (benefit maximum) Prenatal nutrition visit limit reached Check for more visits under a medical diagnosis such as GDM

Every diagnosis code dietitians use, including the other pregnancy codes, is in the ICD-10 codes for dietitians reference.

Frequently asked questions

What is the ICD-10 code for obesity in pregnancy?

O99.21-, obesity complicating pregnancy, childbirth, and the puerperium, with a sixth character: O99.211 first trimester, O99.212 second, O99.213 third, O99.210 unspecified trimester, O99.214 childbirth and O99.215 puerperium. Add an E66 code for the type of obesity.

Can I use a BMI code for a pregnant patient?

No. The Official Guidelines say not to assign Z68 BMI codes during pregnancy. Document the weight and BMI in your note, but leave Z68 off the claim.

Which goes first, O99.21- or E66?

O99.21-. Chapter 15 codes have sequencing priority, and the E66 category says to code first O99.21- when obesity complicates pregnancy. The E66 code follows to describe the obesity, such as E66.812 for class 2.

How do I pick the trimester character?

From the provider's documentation of the trimester or weeks for that encounter. First trimester is less than 14 weeks 0 days, second is 14 weeks 0 days to less than 28 weeks 0 days, and third is 28 weeks 0 days until delivery.

What code do I use postpartum?

O99.215, obesity complicating the puerperium, during the postpartum period, which the guidelines define as the six weeks after delivery. After that, obesity is coded with E66 and a Z68 BMI code again.
