Z71.3 (dietary counseling and surveillance)

DefinitionThe ICD-10-CM code for dietary counseling and surveillance: it describes why the visit happened, not a disease, and payers treat it very differently.

Z71.3 is an ICD-10-CM Chapter 21 code: it records the reason for the encounter (dietary counseling), not a medical condition. It is valid for FY 2026 and unchanged in the FY 2027 code set effective October 1, 2026 (CMS ICD-10 files).

The tabular list attaches two instructions to Z71.3: use an additional code for any associated underlying medical condition, and use an additional code for BMI (Z68.-) if known.

How payers use it varies, which is why it causes so many denials:

Ask on every verification call which position the plan wants for Z71.3, and code only what the record supports.
